This position performs interventions under the Health Education and Risk Reduction (HE/RR) contract from the Los Angeles County Department of HIV & STD Programs. (100% contract requirement) along with other community outreach efforts. Key responsibilities include meeting the Scope of Work as outlined by both programs and contract, administering program-related activities, care navigation and linkage to care, case management of social services in the continuum of care, reducing client falling out of care, working with over 100 partner social service agencies to keep client-members engaged in care and successful in their health outcomes, working with mental health providers to ensure clients receive necessary support, educate client-members on U=U (Undetectable equals Untransmissable), conducting community outreach, operations, program planning and evaluation and to make an impact on those affected by HIV, STDs, risk reduction behaviors and HIV prevention through PrEP outreach.

-High School Diploma or GED required
-College Degree (public health, psychology, social work or education preferred)
-Experience in related field including the nonprofit sector
